Evaluators,

We thank you for helping us to evaluate members of CFTOA. As a professional organization, these evaluations help our member officials improve their officiating skills and competency.

Please complete the two page form as completely as possible and provide personal feedback in your evaluation.

Also, please make recommendations in the areas that need the most improvement. When the evaluation form is complete, please scan and email the completed form to Lionel Bonck, the chairman of the Professional Review Committee.

It may also be helpful to have a conversation with Lionel if additional information is needed.
